We are here to help you through every step.
- First of all you need to attend an information seminar. To register you can call the office at 937-208-5300 or sign up here on line.
- Sign up for a consult with one of the surgeons at the end of the seminar.
- Contact your insurance company and check your benefits for weight loss surgery.
- Have your primary care physician send a letter stating that He/She thinks you would be a good candidate for surgical weight loss. (Download form for family physician on this web site.)
- Make an appointment for your nutritional assessment. Call 937-208-2353 to set that up.
- Make an appointment with the psychologist. (List will be provided at the seminar.)
- Attend a Support group meeting. You can call 937-208-4968 and select option#2 for information about upcoming meetings or click on Support group for the dates and times.
